If you are adding dependents to your coverage you will need to provide legible copies of the supporting documentation, such as: Copy of Social Security Card, Long Form Birth Certificate, Marriage License or page 1 of your federal tax return, etc. to your Benefit Administrator Vickie Hutchins at vhutchins@spart7.org . *Your enrollment coverages will not be fully processed without the required supporting documentation.*

 

If you have not viewed 100% of your new hire orientation presentation, please do so before your scheduled new hire benefits enrollment appointment.
